<dc:title>Pre-operative determination of Carcinoembryonic Antigen (p-CEA) as an independent prognostic factor of Disease Free Survival (DFS) in patients treated for colorectal carcinoma</dc:title>

<dc:description>OBIETTIVO: Valutare, in un gruppo di pazienti trattati radicalmente per carcinoma colorettale, se la determinazione dell’antigene carcinoembrionario (p-CEA) possa avere una valenza prognostica e costituire un fattore di rischio indipendente in relazione al disease free survival. BACKGROUND: Il p-CEA sembra essere correlato sia alla stadiazione della neoplasia colorettale, sia alla prognosi del paziente, sebbene ciò non sia ancora stato certamente dimostrato e sia argomento di un acceso dibattito nella comunità scientifica. MATERIALI E METODI: Questa è un’analisi retrospettiva di un database prospettico. 395 pazienti sono stati trattati radicalmente per carcinoma colorettale. Il p-CEA è stato confrontato statisticamente con la stadiazione AJCC 2010, I parametri T ed N e il grading. Tutti i dati raccolti sono stati testati relativamente alla loro associazione con il disease-free survival (DFS). I soli fattori associate significativamente (p&amp;lt;0.05) con il DFS sono stati usati per costruire un’analisi multivariata allo scopo di stabilire quali fossero fattori prognostici indipendenti. RISULTATI: Una relazione statisticamente significativa è stata dimostrata tra p-CEA e la stadiazione del tumore (p &amp;lt;0.001), il parametro T (p &amp;lt;0.001) e il parametro N (p &amp;lt;0.006). Nell’analisi multivariata I fattori prognostici indipendenti trovati erano: p-CEA, stadio N1 ed N2 secondo l AJCC e il grading G3. Una differenza statisticamente significativa (p &amp;lt;0.001) è risultata evidente tra il DFS di pazienti con valori di p-CEA normali ed elevate. CONCLUSIONI: IL p-CEA sembra essere in grado di selezionare in fase pre-operatoria quei pazienti per i quali si può prevedere una stadiazione più avanzata.</dc:description>
<dc:description>OBJECTIVE: To evaluate whether, in a sample of patients radically treated for colorectal carcinoma, the pre-operative determination of the carcinoembryonic antigen (p-CEA) may have a prognostic value and constitute an independent risk factor in relation to disease free survival. SUMMARY OF BACKGROUND DATA: The p-CEA seems to be related both to the staging of colorectal neoplasia and to the patient's prognosis although this, to date, has not been conclusively demonstrated and is still a matter of intense debate in the scientific community. METHODS: This is a retrospective analysis of prospectively collected data. 395 patients were radically treated for colorectal carcinoma. The pre-operative CEA (p-CEA) was statistically compared to the AJCC 2010 staging, the T and N parameters, and grading. All parameters recorded in our database were tested for an association with disease-free survival (DFS). Only factors significantly associated (p&amp;lt;0.05) with the DFS were used to build multivariate stepwise forward logistic regression models to establish their independent predictors. RESULTS: A statistically significant relationship was found between p-CEA and tumor staging (p &amp;lt;0.001), T parameter (p &amp;lt;0.001) and N parameter (p = 0.006). In a multivariate analysis, the independent prognostic factors found were: p-CEA, stages N1 and N2 according to AJCC, and G3 grading (grade). A statistically significant difference (p &amp;lt;0.001) was evident between the DFS of patients with normal and high p-CEA levels. CONCLUSIONS: P-CEA makes a pre-operative selection possible of those patients for whom it is likely to be able to predict a more advanced staging.</dc:description>
