CONFRONTO DEI RISULTATI VISIVI E TOPOGRAFICI IN PAZIENTI CON CHERATOCONO SOTTOPOSTI A CROSS-LINKING CORNEALE: TECNICA TRADIZIONALE Vs TRANSEPITELIALE

ABSTRACT
Obiettivo. Confronto dei risultati visivi e topografici in pazienti con cheratocono in evoluzione, sottoposti a cross-linking corneale (CCL) con procedura epi-off ed epi-on.
Materiali e metodi. 72 pazienti; 36 tecnica standard e 36 tecnica transepiteliale, da gennaio 2010 a luglio 2011. Pachimetria > 400 ìm. Follow-up dopo trattamento (1,3 e 6 mesi) eseguito con: visus naturale e corretto, refrazione, biomicroscopia alla lampada a fessura, topografia corneale.
Risultati. 36 trattati con CCL standard dopo 6 mesi si dimostra riduzione del Kmax di 2 D e aumento visus di 3/10. Complicanze: 4 casi haze transitorio a 2 mesi risolto con steroidi topici. 36 con CCL transepiteliale, incremento visus di 5/10 e riduzione Kmax di 4 D e no complicanze significative.
Conclusioni.CCL transepiteliale migliore di quello standard (riduzione Kmax e aumento visus). Considerando l assenza di complicanze postoperatorie, meglio adoperare il CCL epi-on in questi casi.
ABSTRACT
Purpose. Comparison of visual and topographic outcomes between patients with evolutive keratoconus treated with standard or transepithelial corneal cross-linking (CCL).
Materiali e metodi. 72 patients; 36 standard procedure e 36 transepithelial one, from January 2010 to July 2011. Pachimetry > 400 ìm. Follow-up after treatment (1,3 e 6 months)
Risultati. 36 treated with standard CCL after 6 months Kmax reduction of 2 D, visual improvement of 3/10. Complications: 4 cases of transient haze after 2 months treated with topical steroids. 36 treated with transepithelial CCL, visual improvement of 5/10 and Kmax reduction of 4 D and no evident complications.
Conclusioni. Transepithelial CCL better than standard one (Kmax reduction and visual improvement). Considering the absence of postoperative complications, it is better to treat patients with transepithelial CCL.
