Surgical decision-making in cases of post-traumatic CVJ injuries is challenging because the spinal insult may involve bony structures, ligaments, or both. Even though most of the injuries may be treated conservatively with external immobilization in collars and early rehabilitation, there are some specific clinical and radiological scenarios requiring surgery. The aim of this chapter is to elucidate the surgical strategies indicated in patients needing surgical open reduction and fixation by means of fully illustrated cases. The conservative approach to CVJ traumatic lesions will not be a topic of this chapter.

Abstract
