Le indicazioni al trattamento chirurgico della retto-colite ulcerosa

The indications to surgical treatment of the ulcerative colitis (RCU) are a discuss argoment to date, for etiopathogenetic uncertains still being. The problems of treatment are expressed by A. G. Parks and J.H. Pemberton very well, who said that «... Its treatment, both medical or surgical, is empirical. », and «... The patients with cronic ulcerative colitis heal with proctocolectomy». From 1980 to 1992 we observed 65 cases of RCU. 21 (32%) of these were treated by surgery. The indicatiom to surgical treatment were: unsuccesfull of medical therapy (14 cases), toxic megacolon (3 cases), severe dysplasia (2 cases), perforation (2 cases). We have submitted 5 patients to colectomy with ileostomy, 13 patients to colectomy with ileus-rectoanastomosis and 3 to proctocolectomy with definiteve ileostomy. Morbility was 14,2% (anastomotic linkage, bronco pneumonia sepsis, wound sepsis). Mortality was 4,7% for a case of large bowel multiple perforations. The treatment of the RCU is a difficoult choice for sourgeon, because he must decide the surgery timing. To different of the Crohns disease, that is not heal with surgery, today RCU allows more surgical aggressive attitude.

Le indicazioni al trattamento chirurgico della RCU (rettocolite ulcerosa) costituiscono a tutt'oggi un argomento controverso e risentono delle incertezze etiopatogenetiche e nosografiche ancora esistenti su questa malattia. I termini della problematica del trattamento medico e chirurgico sono bene espressi da due celebri cultori della materia, quali Parks e Pemberton, che affermano che il trattamento è empirico e che solo la chirurgia con la proctocolectomia è in grado di guarire definitivamente il paziente colpito da RCU. A differenza della malattia di Crohn (a cui caratteristiche cliniche e anatomopatologiche strettamente la correlano sul piano nosografico) che non è guaribile con la chirurgia, si ammette oggi nei confronti della Reu un atteggiamento chirurgièo più aggressivo, anche se dopo provato insuccesso della terapia medica.
