Trocars insertion in laparoscopic surgery may be responsible for various complications with a frequency ranging from 0.1 to 1% as reported in the medical literature. After a brief survey of different first trocar insertion techniques and methods to achieve pneumoperitoneum, the advantages offered by an 'open technique' are underlined. Because of its safety, it is suggested that this technique should be considered the standard method to obtain initial pneumoperitoneum in laparoscopic procedures
