A 75-year-old man underwent endoscopic ultrasoundexamination for abdominal pain and jaundice. At theend of the examination, the patient presented severe epigastricpain; CT scan showed a 7-cm area of dense liquid behind theposterior gastric wall and ischemia of the spleen. Splenectomyand cholecystectomy were carried out 7 days after thesymptoms. It was hypothesized that a little difficulty of thepassage of the echoendoscope through the pylorus could havecaused a small injury of the splenic vessel determining theextraluminal bleeding. Extraluminal hemorrhage following aconventional endoscopic ultrasound examination has notbeen described previously.
