A 75-year-old man underwent endoscopic ultrasoundexamination for abdominal pain and jaundice. At theend of the examination, the patient presented severe epigastricpain; CT scan showed a 7-cm area of dense liquid behind theposterior gastric wall and ischemia of the spleen. Splenectomyand cholecystectomy were carried out 7 days after thesymptoms. It was hypothesized that a little difficulty of thepassage of the echoendoscope through the pylorus could havecaused a small injury of the splenic vessel determining theextraluminal bleeding. Extraluminal hemorrhage following aconventional endoscopic ultrasound examination has notbeen described previously.

Un homme de 75 ans a été examiné par échoendoscopiegastrique suite à une douleur abdominale et à un ictère.À la fin de l’examen endoscopique, le patient présentait uneintense douleur épigastrique. Le CTscanner a mis en évidenceune plage de 7 cm, contenant un liquide dense, en arrière de laparoi postérieure de l’estomac et un aspect d’ischémie de larate. Le drainage de la collection hématique, la cholécystectomieet la splénectomie ont été réalisés sept jours après l’examendiagnostique. L’hypothèse est qu’un passage difficile del’échoendoscope à travers le pylore, secondaire à un phlegmonpérivésiculaire, pourrait avoir causé la lésion d’unvaisseau splénique et donc un saignement extraluminal.Cette complication durant l’endoscopie conventionnelle n’ajamais été décrite auparavant.
