Purpose: to summarize the update Literature on management of posterior vaginal compartment defect. Methods: A Medline search of the most relevant reviews regarding rectocele repair, was done. Thirty-seven important works were examined. Results: Literature research showed that transvaginal is more efficient than transanal approach to repair the posterior compartment defect. The Denonvilliers’ recto-vaginal septum is better restored with anatomic modulation by means of the vaginal route. The transanal approach is associated with more complications: rectal symptoms, infection, fistula. Anatomical recurrence rate is poorer after transvaginal surgery.
