Given the growing attention to changes in performance measurement and evaluation systems in healthcare contexts, the present study aims at improving our understanding of such change processes within teaching hospitals, examining how managerial and health-professional logics contribute to these changes. In the theoretical part of the study we analyze teaching hospitals as multi-stakeholder contexts. Particularly, we propose a theoretical approach that represents changes as dialectical phenomena (Van de Ven, Poole, 1995) so as to explain how the interaction between influential stakeholders, representing managerial and professional logics, affects changes in performance measurement and evaluation systems. The empirical part of the paper is devoted to a case-study on changes in performance measurement and evaluation systems within a Sicilian teaching hospital. The empirical analysis aims at examining the observed changes drawing on the theoretical framework proposed, emphasizing the interactions between the logics characterizing the teaching hospital context.
Alla luce dell’attuale interesse per i cambiamenti nei sistemi di misurazione e valutazione manageriali nei contesti sanitari, questo studio si propone di migliorare la comprensione di tali processi di cambiamento nelle aziende ospedaliere universitarie, indagando il modo in cui la logica manageriale, da un lato, e quella medico-professionale, dall’altro, contribuiscono a tali cambiamenti. Nella parte teorica del lavoro si analizzano le aziende ospedaliere universitarie come contesti multi-stakeholder. In particolare, si ricorre ad un approccio teorico che interpreta il cam-biamento come fenomeno dialettico (Van de Ven, Poole, 1995) per esplicare come l’interazione tra gli stakeholder influenti, rappresentativi delle due logiche, condiziona i cambiamenti nei sistemi di misurazione e valutazione. La parte empirica è dedicata ad un caso-studio sui cambiamenti nei sistemi di misurazione e valutazione delle performance manageriali in un policlinico universitario siciliano. Lo studio empirico si propone di analizzare tali cambiamenti mediante il framework teorico proposto, mettendo in evidenza l’interazione tra le logiche presenti all’interno della struttura ospedaliero-universitaria.
