The Authors report a case of overwhelming postsplenectomy sepsis risen in a patient splenectomized twenty years before due to Cooley's disease. A careful review of the literature has allowed a new trend of strategy to adopt in the surgical treatment of splenic diseases. They observed that literature deals especially in traumatic pathology, where overwhelming postsplenectomy sepsis (OPSI) mortality is very low; on the contrary poor attention has been dedicated to OPSI prophylaxis in hemopathic patients, where mortality is higher. Therefore, it is important to evaluate carefully common low respiratory tract infections and to supply hospital assistance in presence of fever in order to reduce the complications of this serious syndrome.
