Gallbladder volvulus is considered a rare clinical condition, its clinical observation is estimated to be on about one case every 365,000 hospital admissions. From its first description in 1898 by Wendel, about 500 cases have so far been reported over about 120 years of surgical literature describing a variety of clinical presentations, 1 confirming the rarity of observation even during a long surgical career. We report a very rare case of gallbladder torsion located in a Spigelian hernia sac, mimicking a strangulated Spigelian hernia, i.e., to best of our knowledge, the only one reported to date in the scientific literature.
