Splenic artery aneurysms are the most frequently reported form affecting the splenic district. A higher incidence is present in pluriparas and patients suffering from portal hypertension. Surgery consists of the excision of the aneurysm accompanied by splenectomy in the majority of cases. The authors report a case of the flattening of a 3-cm splenic artery aneurysm and the subsequent direct reconstruction of arterial continuity. They affirm that it is preferable to conserve the spleen owing to its important immune functions. Arterial reconstruction may be safely accomplished in the event of marked tortuosity of the splenic artery. Control colour Doppler ultrasonography at 40 months confirmed the patency of the splenic artery and the morphology of the spleen was normal.
