Aims. Few data in the literature to clarify the true prevalence of sexual male accessory glands inflammations, registered in the diabetic population (DM). Also just-depth are the real pathogenetic mechanisms underlying this association. In a selected population of diabetic patients (DM) with primary infertility and associated clinical condition of sexual male accessory glands infection (MAGI), description of conventional sperm parameters, the concentration of leukocytes and free radicals of oxygen in sperm, the ultrasound andrological characterization, compared to a group of infertile patients with only MAGI. Materials and Methods. The study was conducted on 16 patients with type II DM, average age 37 ± 3 years (27-45 years) with MAGI (WHO criteria, 1993), and primary infertility; 30 patients, average age 33 ± 4 (21-45 years) with MAGI (WHO criteria, 1993), and primary infertility. Results. The concentration of seminal leukocytes and oxygen free radicals in baseline and after stimulation with specific inducer (f-MLP) in chemiluminescence, in fractions of Percoll 45 and 90% was significantly increased in the group with DM and asociated MAGI. The ultrasound evaluation showed impaired emptying of the ampullarvescicular-ductal tract, in the immediate post ejaculatory. Conclusions. MAGI is a clinically relevant model of spermatic oxidative stress. Our study shows that DM combined with MAGI worsens leukocyte and free oxygen radicals concentration in sperm, without altering the conventional parameters. Also DM seems to favor extension and maintenance of the inflammatory process
