AIM OF THE WORK. The purpose of this work is to show the proper approach in implant dentistry when basic requirements for primary stability are lacking and long-term success is not sure. DISCUSSION. Several classification are available to help clinicians during patient evaluation. After a brief exposition of the main surgical techniques, the Authors report two cases treated by Summers technique together with the description of patient evaluation before surgery to choose the best option and the characteristics of each procedure. RESULTS. The gold standard is represented by Summers' technique, especially when a greater bone increase is needed.
